New Benefits Information: CalPERS Health

Dear Employees:

In late September, you should receive the attached open enrollment materials and enrollment form in the mail regarding the District’s change to CalPERS Health. The effective date for this change will be January 1, 2017. Your current medical benefits through Kaiser or Health Net will end on December 31, 2016. The Open Enrollment period is October 3, 2016 through October 31, 2016.

YOU MUST TAKE ACTION TO CONTINUE OR TO NEWLY ENROLL IN MEDICAL COVERAGE

Informational meetings and regular open enrollment will be held during the month of October in the District’s Board Room – 1231 Addison Street. Due of the large volume of anticipated employees, the information sessions have been divided alphabetically by the employee’s last name on the dates below:

  • October 6, 2016 from 1:00 pm to 5:30 pm, A – K (District Board Room)
  • October 11, 2016 from 1:00 pm to 5:30 pm, L – Z (District Board Room)
  • October 18, 2016 from 1:00 pm to 5:30 pm, A – Z (Berkeley High School)

Additional assistance will also be available during the District’s regular Open Enrollment hours to be held almost daily on the dates and times listed below:

  • October 3 ‐ October 5: (Monday through Wednesday, Board Room) 8:30 am—5:00 pm
  • October 12 ‐ October 13:(Wednesday through Thursday, Board Room) 8:30 am—5:00 pm
  • October 17: (Monday, Board Room) 8:30 am—6:00 pm
  • October 19 ‐ October 20: Wednesday through Thursday, Board Room) 8:30 am—6:00 pm
  • October 24 ‐ 26: (Monday through Wednesday, Board Room) 8:30 am—6:00 pm
  • October 27: (Thursday, Room 126, 2020 Bonar) 8:30 am-6:00 pm
  • October 31 ‐ Last Day: (Monday, Board Room)7:30 am—6:00 pm

YOU MUST TAKE ACTION TO CONTINUE TO CONTINUE OR TO NEWLY ENROLL IN MEDICAL COVERAGE

As an employee eligible for medical insurance through the District, you will be required to notify the District of either your election or your declination of enrollment into one of the CalPERS medical plans.

In addition, if you are planning to enroll dependents, you will be required to supply dependent verification:

  • Marriage Certificate/Certificate of Declaration of Domestic Partnership filed with the California Secretary of State or affidavit of domestic partnership and/or
  • Birth Certificate(s) of your child(ren).
  • Enrollment of dependents will not be allowed without verification, so please plan on obtaining this information prior to open enrollment and have copies ready to submit with your enrollment application.

    How to Submit Your Mandatory CalPERS Forms & FSA Election Forms

    Please review the “Open Enrollment Checklist” on page 5 of the BUSD Active Employee Guide to District Health Benefits Sept 2016.  All eligible employees must submit a CalPERS form.
    You may turn in your completed enrollment/change and/or election forms directly to the Office of Risk Management/
    Benefits Department by:  

    1. Walk‐in: Employees may submit the completed forms during window hours (see above for hours). Employees can walk‐in completed forms until 6:00 pm on October 31, 2016.
    2. Mail: Employees may submit the completed form(s) through Postal Mail. Forms must be received no later than 6:00 pm on October 31, 2016. Postmarked submiƩals received after this date will not be accepted.
    3. E‐Mail: Employees may submit the completed form(s) through E‐mail. Please e‐mail to openenrollment@berkeley.net. Forms must be received no later than 6:00 pm on October 31, 2016. E‐mailed submittals received after this date will not be accepted.
