Vaccine Consent Form – Law requires parental consent for all students under 18 receiving the Tdap Immunization. Any BHS student with a signed consent form can visit the BHS Health Center at any time school is open to get the vaccine. You can find other free clinic information here. Students who have not received the Tdap immunization will be unable to attend school starting on Thursday morning, September 29th. Further links are listed below.
Tdap (Whooping Cough) vaccination mandatory for secondary students
This coming school year (2011-2012), all students entering into 7th, 8th, 9th, 10th, 11th or 12th grades will need proof of an adolescent whooping cough booster shot (called “Tdap”) before starting school. Contact your health care provider to find out if your child needs the “Tdap” booster shot to meet the new requirement. If you do not have a regular provider, the vaccine is offered through the Immunization Program at the City of Berkeley through the Public Health Clinic. BUSD families with students grades 7-12 are required to bring proof of the TDAP booster to the school(s) your child(ren) attend(s) before they can begin school next year.
